About Us

Person Centered Care is a private Home Health Care Agency established by Kelly Youssef and Karishma Patel.  Together, the co-founders had the vision to help enable people, particularly those with developmental disabilities, to achieve the highest level of a quality, inclusive life.  Between them, Karishma and Kelly have over 20 years of experience helping those individuals to experience the satisfaction of being able to self-direct their lives.

We partner with individuals, their loved ones, and their caregivers to help create an individualized plan to meet the person’s needs. The Direct Support Professionals that we recruit, hire, train, and manage are specifically placed on a team where their skills, interests, and background will be best suited to the individual being supported.  Once in place, this team is there to support the individual in their own home or within their family home, as well as helping them to establish a valued presence in their community.  Families can put their minds at ease, trusting that now and in the future, we are there to ensure that their loved ones will continue to live the life that they want and deserve.
